Matrix Home Care is hiring licensed Physical Therapists and Occupational Therapists to provide skilled home health therapy services throughout Hillsborough County, Polk County, and surrounding communities.

This is a PRN/per-diem opportunity offering flexible scheduling and the ability to accept patients based on your availability and preferred service area. Therapists will provide individualized care in patients’ homes while collaborating with physicians, nurses, caregivers, and other members of the interdisciplinary care team.

Compensation

Pay: $75–$125 per visit

Position Responsibilities
  • Complete comprehensive physical therapy or occupational therapy evaluations in patients’ homes.
  • Assess each patient’s functional abilities, limitations, safety concerns, home environment, and rehabilitation needs.
  • Develop individualized plans of care based on physician orders, clinical findings, and the patient’s goals.
  • Provide skilled therapy treatments and follow-up visits in accordance with the established plan of care.
  • Help patients improve mobility, strength, balance, endurance, coordination, safety, and independence.
  • Assist patients with regaining or maintaining the ability to complete activities of daily living.
  • Recommend adaptive equipment, assistive devices, and home-safety modifications when appropriate.
  • Develop and provide individualized home exercise programs.
  • Educate patients, family members, and caregivers regarding treatment techniques, safety precautions, equipment use, fall prevention, and continued exercises.
  • Monitor patient progress and adjust treatment interventions as clinically appropriate.
  • Communicate changes in the patient’s condition, progress, concerns, or barriers to the physician and interdisciplinary care team.
  • Complete all required clinical documentation accurately and within established timeframes.
  • Participate in care coordination and discharge planning.
  • Follow agency policies, professional standards, infection-control procedures, and applicable state and federal home health regulations.

Requirements

Physical Therapist Qualifications
  • Current and unrestricted Florida Physical Therapist license.
  • Graduate of an accredited physical therapy program.
  • Home health, rehabilitation, hospital, outpatient, skilled nursing, or related clinical experience preferred.
  • Experience completing home health evaluations and treatment visits strongly preferred.
  • Knowledge of therapeutic exercise, gait training, transfer training, balance activities, fall prevention, and mobility equipment.
  • Ability to work independently and manage an assigned patient schedule.
  • Strong assessment, communication, documentation, and patient-education skills.
Occupational Therapist Qualifications
  • Current and unrestricted Florida Occupational Therapist license.
  • Graduate of an accredited occupational therapy program.
  • Home health, rehabilitation, hospital, outpatient, skilled nursing, or related clinical experience preferred.
  • Experience completing home health evaluations and treatment visits strongly preferred.
  • Knowledge of activities-of-daily-living training, upper-extremity rehabilitation, adaptive equipment, energy-conservation techniques, and home-safety recommendations.
  • Ability to work independently and manage an assigned patient schedule.
  • Strong assessment, communication, documentation, and patient-education skills.
Additional Requirements
  • Current CPR certification.
  • Valid driver’s license.
  • Reliable transportation.
  • Current automobile insurance.
  • Ability to travel to patients’ homes within the selected service area.
  • Reliable access to a phone, computer, or tablet for communication and documentation.
  • Ability to complete documentation accurately and on time.
  • Professional liability insurance, if required.
  • Ability to satisfy all employment, credentialing, health, and compliance requirements.
Background-Screening Requirement

Under Florida law, this role is subject to background screening pursuant to Chapter 435, Florida Statutes , including fingerprinting and review for employment eligibility before beginning work.
